The Animal Hospital at Steiner Ranch offers our registration forms online so you can complete them at home or work.
- Download and open the forms with Microsoft Word or other common word processing applications.
- Preferred option is to type your information into the shaded areas of each form, save the modified documents to your computer as .DOC, .DOCX, or .PDF files, then email to "[email protected]" 48+ hours prior to your pet's first appointment. Please DO NOT save and return the forms as .PAGES files!
- A second option is to print the blank forms and fill them out by hand. You can physically bring or scan & email the completed forms to us, at least 24 hours prior to your appointment.
Registration Forms (Download and complete all three; we will not need a new Client Registration if you are an existing client adding a new pet)
The Client and Patient registration forms are used to set up a record for you and your pet(s). The Records Release serves two purposes: (1) It allows us to send your pet's records to specialists, emergency hospitals, boarders, groomers, or trainers in the future if needed; (2) it authorizes us to obtain your pet's medical records from any prior veterinarians.Also bring whatever pet medical records you have to the first appointment, in case they are delayed arriving from your previous veterinarian(s). We will scan the pages we need and return your originals to you.
